Mailing Address Change Instructions
IMPORTANT: If you are requesting a mailing address change and are not the property owner, additional documentation is required before your request can be processed. Please provide one of the following:
- Deceased Owner: Include a copy of the Death Certificate.
- Power of Attorney: Include a copy of the POA document authorizing the request.
- Business-Owned Property: Include the Articles of Incorporation showing your authority to request the change.
Please complete the required information on the Mailing Address Change Request form and submit it using one of the following methods:
